16 & Under

November 16, 2003 * 2:00-4:00

17 & Under

WHAT TO BRING:

1. USAV forms (Individual Registration and Medical Release * these forms must be signed by a parent) as well as the Dallas Summit Tryout form. You can download these forms from our website at www.dallassummit.com, or the forms will be available at the tryouts.

2. $50.00 tryout fee. This fee will cover all tryouts and each participant will receive a free T-shirt.

Candidates should check-in 30-45 minutes prior to their tryout time to fill out forms and stretch. We encourage candidates to attend all of their respective tryouts if possible.

All Tryouts will be held at the Summit Sports Complex. For directions visit our website at www.dallassummit.com as well.
